About Your Visit

If you are a new patient you will be asked to fill out the New Patient Forms & HIPAA Privacy Policy at your first visit. For your convenience please download, complete and bring the completed forms with you to your appointment. If you have any questions or would like to request that the forms be mailed to you please contact us at (510) 647-1200.

 

At the Time of Your Appointment Please Bring:

  • Photo ID
  • Insurance Cards
  • All Diagnostic Tests – i.e., MRI, X-rays, CT scan
  • A list of all current medications
  • Medical Records from other Treating Physicians
  • Co-payments

Insurance & Billing
The Pacific Bone and Joint Clinic currently contracts with the following insurance companies and forms of payment:

  • Medicare – Participating Providers
  • PPO – Blue Cross, Blue Shield, Cigna, Sutter Select, United Healthcare, Others
  • Commercial Insurances
  • HMO (Brown and Toland, Alta Bates Medical Group, Sutter East Bay Medical Foundation)
  • Credit and Debit Cards (Visa, Mastercard), Cash, Check

To determine if we participate in your insurance plan, please call your insurance company or look in your participating network guide prior to your visit.

We collect all co-pays and outstanding balances at the time of visit.
